Spray Tan Consent Form
Clients under the age of 18 must be accompanied by a parent or guardian The spray tanning process is accomplished
by the application of a   solution that contains the active ingredient, DHA (Dihydroxyacetone)DHA has been approved
in cosmetic use by the FDA.  The DHA solution is sprayed onto the skin manually with a HVLP (High Velocity Low
Pressure) spray gun.

You should wear a dark swimsuit or underwear.  The solution will wash out of your clothing.  For the best results in
removing any solution that might get on your clothes, is to wash the clothing as soon as possible.

The spray tanning process will take approximately 5 to 15 minutes depending on what areas of your body are being
sprayed and how much of your body is being sprayed.  Excessive sweating, showering and/or swimming within 8 hours
of the tanning service is not recommended.

All of the products used in the spray tanning procedure are generally regarded as safe.  Be advised that ALL PEOPLE
ARE DIFFERENT and there are occasions where someone may be allergic to one or more of the ingredients in the
spray tanning solution.  Be advised that there is a small percentage of people whose skin will not react favorably to
spray tanning. Warning  Be advised that if a reaction occurs, you should discontinue the use of spray tanning and
consult a physician.

Warning - The safety of this product has not been fully tested.

Warning - The spray tanning solution does not contain an active sun screen.  Therefore your spray tan will not protect
you against the sun.  The solution contains an SPF of 0.

I understand the spray tanning process and hereby agree to allow a Feistan employee to apply a spray tan solution to
my skin.  I have no medical condition or allergy which would preclude me from having this procedure done.  I agree to
hold Feistan Inc. Harmless of any and all medical complications that may arise. I have been advised to discontinue use
if any reaction occurs and to obtain medical attention.

I understand and agree that the person being airbrush spray tanned will not have any type of home
self tanning spray, creams or lotions on their body at least 10 days prior to their scheduled Feistan
airbrush spray tanning appointment.
